Payment and Cancellation Policy

Payment Agreement

This agreement indicates that you have read, understand, and agree to the following payment
policies.

  • You agree to be financially responsible for all charges.
  • You acknowledge that Alpha-Links Inc does not bill health insurance for some of their
    services and does not participate in most health insurance plans at this time but may elect
    to participate in HSA in the future.
  • If using HSA as your form of payment, please note that if your insurance company
    declines to cover our services rendered, an invoice will be issued and the credit card on
    file will be charged.
  • Where applicable you consent that Alpha-Links Inc bill Medicare, Medicaid and other
    healthcare insurance for your services.
  • Payment is expected at the time of service unless you have made other payment
    arrangements with us.
  • You are ultimately responsible for the cost of all treatment, goods, and services provided,
    and if the payment form used is declined, an invoice will be issued and the credit card on
    file will be charged.
  • You may also be charged additional fees for non-payments, returns unpaid payments
    instruments.
  • You will be charged a 25% late fee for non-payment after 30 days, this may increase with
    time. Rate increase with increase in the time/days

Cancelation Policy

  • All Cancellations must be made 24 hours in advance.
    • Cancellations made with less than 24 hours’ notice will be subject to a 50% cancellation fee.
    • No refund if you did not show up for appointment
